Job Summary:

This position is responsible working with the young adults who have been identified as being at very high risk of becoming involved in gun violence. Intensive Case Managers work primarily with young men age 18-35 who have extensive criminal justice involvement, are members of gangs, and have been shot before and/or have close friends who have been shot recently. Intensive Case managers help reduce gun violence in the city by performing the following duties.

Essential Duties

1. Conduct outreach in the community; locate individuals identified as very high risk in order to engage them and enroll in services, respond on-site to gun violence hospitalizations and provide support to victims and families

2. Establish and maintain a positive and trusting relationship with each client and use influence to help clients make better decisions including desisting from violence

3. Case management: develop Life Plans with each client and document all case management efforts, update individual case files, and make all required entries into the data management system.

Job Duties

- Make immediate contact with each referred client once assigned by their supervisor, no later than 24 hours after assignment

- Provides immediate crisis response to POC victims of community violence and their families

- Responds to Portland-metro area hospitals to support POC victims of community violence and their families following an incident

- Conduct outreach in the community to locate individuals identified as very high risk in order to engage them and enroll in Intensive Case Management services

- Frequent travel to different parts of the city and surrounding areas

- Connect with each new client in-person within 48 hours of being assigned

- Have an initial sole focus on establishing a positive and trusting relationship with each client, focused on spending in-person time

- Develop Life Plan (case plans) with each client

- Maintain a positive relationship while also beginning to connect clients to needed services once they are willing and ready

- Use the positive and trusting relationship with each client to begin to influence their decision making in order to improve their outcomes.

- Use the City-s incentive program to provide stipends and other incentives to clients to reward their achievement of milestones in the Life Plan and to support their continued engagement

- Maintain consistent contact with each client on the case load in compliance their phase, as detailed in the contract

- Document case management efforts, update individual case files, and make all required entries into the data management system.

- Attend all mandatory and necessary meetings and trainings, including weekly staff meetings, your team meetings, Case Conferences, and all other meetings and trainings.

- Engage in ongoing trainings about effective gun violence reduction strategies as well as professional development

- Maintains punctual, regular and predictable attendance

- Works collaboratively in a team environment with a spirit of cooperation

- Respectfully takes direction from program manager and coordinator

- Other duties as assigned

Education & Experience

Demonstrated knowledge and understanding of the population served, the dynamics of the city-s gun violence problem and the groups and individuals who are driving the violence. Willingness to navigate challenging neighborhoods, people, and families. Formal education or training in case management, social work, conflict resolution, violence reduction or criminology preferred but not required.

Certificates, Licenses and/or Registrations

A valid driver license and the ability to meet insurability requirements.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write reports and business correspondence. Ability to speak effectively before groups of clients, employees or external stakeholders.

Mathematical Skills

Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.

Computer Skills

Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

The employee must occasionally lift and/or move up to 25 pounds. While performing the duties of this job, the employee is regularly required to sit, walk, use hands to finger, handle, or feel and talk or hear. The employee will regularly work in outdoor weather conditions that could be wet or humid, extremely hot or extremely cold. The employee will also regularly work in close proximity to firearms, and in situations where there is a risk of violence. The noise level in the environment is moderate to loud.
