Summary:

This position is responsible for serving gang-impacted African American boys (aged 12-18) and their families with multiple needs and barriers. This position also helps disconnected African-American males reintegrate back into their communities and stay focused on positive aspects of their lives by performing the following duties.

Essential Duties:

1. Manages a caseload of gang-involved male youth or youth who are at risk for gang involvement and who are on probation.

2. Provides weekly contact and visits with youth in the school, home, detention, residential treatment facilities, OYA, and in the community.

3. Supports youth in developing positive coping skills from a strengths-based perspective, and in making positive choices and develop positive decision-making skills.

Responsibilities


  • Develops trusting relationships with assigned youth and their families built on client-centered goals linked to their probation service plan and integrated to CHI menu of services.
  • Exposes youth to pro-social activities and transport youth to appointments that support youth-s goals in probation completion and community engagement.
  • Exposes youth to activities and opportunities through critical mentoring-focused on the development of a critical consciousness to understand social, political, and economic oppression.
  • Supports youth with treatment services and community resources, as needed.
  • Empowers youth to advocate for themselves and to develop a positive cultural identity as an African American young man.
  • Attends court, school, treatment, wrap-around, and other meetings as support when requested by multi-disciplinary team.
  • Leads scheduled weekly mentor support group meetings, activities, and events.
  • Maintains client files by documenting contacts and visits with youth, caregivers, juvenile court counselors, and multi-disciplinary team meetings.
  • Attends all trainings and meetings, as needed.
  • Maintains confidential information.
  • Models and encourages direct, professional communication, and appropriate behavior and maintains professional boundaries.
  • Maintains punctual, regular and predictable attendance.
  • Works collaboratively in a team environment with a spirit of cooperation.
  • Respectfully takes direction from manager.
  • Other duties as assigned.

Experience/Education:

  • Associate-s degree required or equivalent and 3+ years demonstrated case management experience-Bachelor-s Degree Preferred.
  • Possesses strong cultural competency and an understanding of social service practices and experience working with families from diverse backgrounds. Experience with crisis intervention, community networking and resource development. Working knowledge of the symptoms of physical and mental health disorders, substance abuse, behavior and adjustment problems of juvenile offenders, and limitations or disabilities in children, adolescents, and adults.

Certificates, Licenses and/or Registrations:

Valid driver-s license with insurability.
