Summary

This position is responsible for providing community-based early intervention for referred youth offenders and their families with the goal of preventing youth from entering the juvenile justice system. 

Responsibilities

        Essential Duties

        1. Provides short-term case management, assertive engagement, trauma-informed care, and crisis intervention to a caseload of youth

        2. Assesses and implements strength-based individual success plans built on culturally specific and evidenced-based practices to help participants develop and reach goals.

               3. Works collaboratively with other professionals in the field including schools, community-based organizations, Mental Health/ Alcohol and Drug organizations, Child Welfare, Washington County Juvenile Department, Washington County Health, Housing and Human Services, and the Oregon Youth Authority


        Job Duties

        • Ensures youth and their families are aware of all community resources available for them and advocate with other service providers for the needs of families as appropriate, including: housing stability, counseling services, and pro-social activities.
        • Provides day-to-day activities including: social casework, intake, assessment, assertive engagement, and skill development through engaging youth and families in activities through: education, employment, home, family, health, pro-social activities, spirituality, and community.
        • Facilitates and/or co-facilitate Restorative Justice Circles, Step-Up, Boys Council, Girls Circle, ART.
        • Participates, develops and coordinates pro-social activities for youth and their families.
        • Adheres to documentation requirements as indicated in CHI Program policies, including accurate data entry into DCJ database, case files, service plans, weekly/monthly reports, and case notes.
        • Ensures timely and detail-oriented data entry, case notes and written reports.        
        • Responds to all crises and risk incidents within the required time frames per Portland OIC policies.
        • Participates in regular supervision check-ins, department staff meetings, community partner meetings, all-staff trainings, and larger POIC initiatives.
        • Maintains punctual, regular and predictable attendance.
        • Works collaboratively in a team environment with a spirit of cooperation.
        • Respectfully takes direction from manager.       
        • Other duties as assigned.

        Education & Experience
        Bachelor's degree from a four-year college or university with three or more years- of demonstrated case management experience. Master-s degree is preferred. Possesses strong cultural competency, understanding of social service practices and experience working with families from diverse backgrounds. Experience with crisis intervention, community networking and resource development.

        Certificates, Licenses and/or Registrations
        This position requires a valid insurable Driver's License.
